Responsibilities:
- Deliver nursing care using focused assessments and established care plans in accordance with UPMC policies, LPN Practice Guidelines, and the State Practice Act
- Communicate patient condition, progress, and changes to the licensed nurse and healthcare team; seek guidance when care needs exceed scope, skill, or patient condition changes
- Administer treatments and perform procedures, including IV therapy, within LPN scope of practice
- Document patient care accurately and thoroughly to support continuity of care
- Provide education to patients and families to support care, discharge, and transitions
- Foster a compassionate, patient-focused environment through Relationship-Based Care
- Participate in quality, safety, and patient experience initiatives
- Support orientation and development of new LPNs and care team members
- Collaborate effectively with patients, families, and interdisciplinary team members to achieve desired outcomes
- Identify and communicate safety concerns and opportunities for clinical or process improvement
- Perform duties in accordance with system-wide competencies and other duties as assigned
- IV therapy curriculum required within 4 months of hire/transfer
- Graduates from 2012 to present: IV therapy included in PA State Board–approved programs
- Graduates prior to 2012: official proof of completion of an approved IV therapy program required
- Ability to practice at the top of LPN license in Med/Surg, Telemetry, or Emergency Department settings
- Ability to collaborate with the healthcare team, patients, and families to achieve optimal outcomes
- Strong interpersonal skills to establish positive working relationships across all levels of the organization
- Physical ability to meet job demands, including standing, walking, lifting, and positioning patients
- Computer proficiency, including use of electronic health records
- Current Licensed Practical Nurse (LPN) license in the state of practice or a multistate license through the Nursing Licensure Compact (NLC)*
- Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification
- Act 34 Clearance
- OAPSA Clearance
